Here's some rules people have been using as long as I can remember:

Jed is dealer, Lydia is on his right, Rachel is on Lydia's right, Kathleen is
between Rachel and Jed.

(an Ace of Spades is currently face up, play is to the right of Jed and it's
Lydia's turn)

Lydia plays an 8 of spades.

Jed says, "Failure to say 8 of spades" and gives Lydia a card.

Lydia takes the card and says "8 of spades".

Jed gives Lydia a card and says, "Failure to say 'I'm sorry'".

Lydia accepts the card and says, "I'm sorry."  (I'm Sorry Rule)

Kathleen plays a Queen of Spades and says "Queen of Spades".

Jed gives her a card: "Failure to say, 'All Hail the Almighty Empress'".  (Tai
Pei Queen Rule)

Kathleen accepts the card and says, "All Hail the Almighty Empress, I'm
sorry."

Jed puts down his last card and says "Mao.  A new rule is in effect."

Lydia plays a 2 of Spades but fails to say anything.

Jed gives her a card: "Failure to say 2 of Spades."

Lydia accepts and says "2 of Spades.  I'm sorry."

Jed gives her 2 more cards and says, "Talking.  Failure to say, 'Thank you sir
may I have another one'."

Lydia accepts them and says, "Thank you sir may I have another one."

Rachel plays a 4 of Spades and says, "4 of Spades.  Last card."

Kathleen plays the other 2 of Spades and says, "2 of Spades.  Mmm, sandwich."

Jed gives her a card and says, "Failure to say "Life, the Universe and
Everything."

Kathleen accepts and says "Thank you sir may I have another one."

Lydia plays a 6 of Spades and says, "6 of Spades."

Rachel plays a 9 of Spades and says, 'Sixty-nine is best naked!  Mao.  A new
rule is in effect."

And so on...
